Two imaging modalities failed to uncover the cause of a patient’s signs and symptoms. An 83-year-old woman with diabetes,hypertension,and a history of gallstones was admitted after 3 days of recurrent vomiting. This was associated with a fever of 100.8° F (38.2° C),hypotension (99/43 mm Hg),leukocytosis,and acute renal failure.
